Love Light Celebration Donation Form

The 24th Annual Love Light celebration gives us a chance to slow down during this busy time of year and remember and honor those friends, family members, and loved ones who are not with us for the holiday season.  Complete the form below with information on how to honor your loved ones. All names honored or memorialized will be printed in the Portage Daily Register special insert that comes out in December. 

In order to have the name of your loved ones included in our program and in the newspaper insert you will need to return the form by November 24^th.  New this year your business can also honor your customers by purchasing lights.
